6. recommendation letter issued by the head of the relevant central government agency in Korea for entertainers: Korea Media Rating Board
for teachers: Ministry of Education
for athletes: Ministry of Culture, Sports and Tourism
7. Diploma and Transcript Copy (for teachers)
8. Visa Fee: php1,500.00


VISA REQUIREMENTS FOR WORKERS (WITH VISA ISSUANCE NUMBER)

1. application form
2. one-passport sized colored photo
3. passport (original and copy)
4. visa issuance number
5. visa fee: php 2,500.00 (single entry)
php 4,000.00 (multiple entry)
6. for F1 and F6 (photocopy of contract)

VISA REQUIREMENTS FOR EMPLOYEES

1. application form
2. 1pc. passport size colored picture
3. passport original (6 months valid)
4. copy of passport first page
5. copy of U.S., New Zealand, Japan, Australia, Korea and Canada Visa (if there is)
6. employment certificate original
7. personal bank certificate original
8. ITR (Income Tax Return) copy

If peronally invited by a Korean: Invitation letter and copy of invitor's passport
If invited by a company in Korea: Invitation letter and copy of Korean Company Business Permit


VISA REQUIREMENTS FOR SEAFARERS (TRANSIT VISA)

1. appplication form (typewritten)
2. one passport size colored picture
3. original passport (with logo of the company)
4. photocopy of the first page of the seaman's book
5. photocopy of the first page of the passport
6. original endorsement letter from manning/shipping company in the Philippines
7. photocopy of the guarantee letter from Korean company
8. photocopy of the overseas employment certificate (OEC)
9. photocopy of employment contract
10. photocpy of medical certificate
11. photocpy of the business permit of Korean agent
